how should you attack a two space extension assuming it is very close to your thickness? Image

No general answer; that depends entirely on each unique board situation.

Probably a typo around "9", missing a co-ordinate letter?

That picture looks like the wrong answer diagram for a fuseki problem.. Also, I don't know what you're talking about because your words seem to have no relation to the picture.

well the source of the diagram isn't necessarily credible but it still doesn't answer how to attack a two space extension. (assuming you are very thick such as in the diagram. Assuming black makes a two space jump towards whites thickness how should white respond? (ie 1 and 2 are played) extending first from thickness doesn't seem like a good option.

Like has been said, it very much depends on the rest of the board, but you usually can't kill outright, even with a lot of thickness around.
