Lost on time

I just read about the tragic incident of the second game of the 50th Oza title match: Cho Chikun lost on time because he didn't realize it was his turn to move.

My question is: How often does it happen, that pros loose on time? Say in a relatively important match with sensible time settings (so Korean 10-minute-TV-matches don't count). Is there a list of such games or even sgf's?
If not, can you name such games?

A search in my (admittedly) outdated version of Gogod returns only 43 games lost on time out of more than 24000 games, the first one in 1926.

One of them, I find...interesting. In the 6th Tengen Challenger's final "Ushinohama fell asleep after the dinner break in one of the 8th floor Nihon Ki-in bedrooms" and lost on time after only 51 moves were played. Does it mean that nobody could find him and wake him up ?

Cho Chikun lost because his stone fell off the board and he didn't get it back in time.

7th Maxim Cup final

Ch'oe Ch'eol-han misunderstood the byoyomi in this fast game. He thought he had three periods of 40 seconds, when in fact he only had one.

8th North American Masters Final

Feng Yun complained about the use of the unfamiliar Ing clock in byoyomi, and other matters, which led to an inquiry and report by the AGA, but the result was confirmed.

I guess it tells us that pros are also humans and sometimes they make blunders. But it seems to be quite rare.
